33 years old Male patient presented for evaluation and treatment of tooth no 1.

Diagnosis: Reversible pulpitis with Asymptomatic apical periodontitis
Treatment plan: Initial Treatment, Retreatment and Apical Surgery.Following anesthesia and rubber dam placement, Conservative access+Ninja access was performed and 3 canals were located.
During treatment, Split Tooth+Vertical Root Fracture+No Cracks/Fractures were identified.
Procedural errors during treatment: I broke an instrument+I gauged the tooth+Didn’t negotiate a canal

Canals were obturated using Cold Lateral+Warm Vertical+Single Cone
Restoration: Temp. rest. placement+Core placement
Treatment was completed in 3 visits.Pre-treatment Mishaps: Broken instrument+Over extension/fill+Missed canal
