48 y.o male patient, ASA I, came with chief complaint related to #20.
Dx: previously initiated, with symptomatic apical periodontitis.
Instrumentation with Vortex Blue 35/04. Irrigation with Sodium
hypochlorite 5.25%. Obturation by cold lateral and ADseal. Not happy
with the obturation (overextended, above the CEJ). Part of the GP was
cut beyond the apex while trying to take the master cone size (Noticed
after that gutta percha was not stored in the fridge since purchased).
Temporized the case after applying Calcium hydroxide. Next visit, I did a
risky move by bending last 1.5 mm of ss H#10 file making it look like a
hook in order to catch the GP somehow. But of course, that 1.5mm got
separated so easily. I didn’t lose hope, third visit, applied the 23
gauge needle down the apex (normal saline) for irrigation and suctioning
(multiple times) and surprisingly the GP and the broken instrument came
out!! Didn’t have the chance to take pictures. Was so excited to
complete the treatment. 4 months follow up.
